Kati Vasti Therapy

Kati Basti is a powerful external oil therapy in Ayurveda that targets the lower back (Kati Pradesh) to alleviate pain, stiffness, and spinal disorders. This therapy involves the retention of warm medicated oil within a dough boundary over the lumbar region, which deeply nourishes the muscles, bones, and nerves.

The term Kati Basti is derived from:
Kati (कटि) – Lower back
Basti (बस्ति) – Retention or reservoir

Kati Basti is widely used to manage Vata disorders, which manifest as back pain, dryness, stiffness, nerve compression, and degenerative spinal conditions. The warmth of the oil enhances blood circulation, muscle relaxation, and joint lubrication, making it beneficial for individuals with chronic lower back pain, sciatica, and lumbar spondylosis.

Procedure

  • Purva Karma (Pre-Treatment Preparation)

    The patient is asked to lie in a prone position (face down) on a massage table. The lower back is gently massaged with a small quantity of warm medicated oil to prepare the area. A dough ring (Basti Kshetra Mala) is prepared using black gram flour or wheat flour and placed around the lumbar region to create a leak-proof boundary.

  • Pradhana Karma (Main Procedure)

    Lukewarm medicated oil is carefully poured into the dough reservoir until it reaches a level sufficient to submerge the targeted area. The oil is kept warm by periodically replacing it with freshly heated oil. The oil is retained for 20–30 minutes, allowing deep penetration and nourishment. During this period, the therapist may gently massage the surrounding muscles to enhance absorption and stimulate circulation. The process may be repeated for 7–14 days, depending on the condition.

  • Paschat Karma (Post-Treatment Care)

    After the oil is removed, the dough is carefully taken off, and the area is gently wiped clean with a warm cotton cloth. The patient is advised to rest for at least 30 minutes. If required, a mild herbal steam therapy (Nadi Sweda) is given to enhance detoxification and circulation. Light, warm, and easily digestible food (Pathya Ahara) is recommended. Excessive physical exertion, cold exposure, and heavy meals should be avoided for a few hours post-therapy.

Contraindications (When to Avoid Karnadhoopam)

  • Severe Ear Infections with Pus (Karna Paka with Srava): Smoke may irritate infected tissue.
  • Perforated Eardrum (Bhinna Karna Patra): Should not be done in case of eardrum rupture.
  • High Fever (Jwara Avastha): May worsen symptoms.
  • Recent Ear Surgery or Injury: Should be avoided during the healing phase.
  • Allergic Reactions to Smoke: May cause discomfort in sensitive individuals.
